Supporting the discharge of elderly people from hospital to reduce readmission 

Age Concern Luton runs a free 7-day hospital discharge project to support people leaving hospital and returning home. The charity says it has a had a really positive impact on reducing hospital readmission rates for social reasons, particularly for older people living alone.

The project involves sending someone to visit elderly people within a short time of discharge and providing support enabling them to readjust to home life. 

Case study: Widowed Bob, 83, had been admitted as an emergency. He was discharged from hospital late evening after a three week stay. Age Concern visited him within an hour of discharge, set up his bedroom, stocked the cupboards and provided the support he needed to mentally readjust to fending for himself again.
